Health Sciences/Nursing Programs Physical Examination: 4.14

ADMINISTRATIVE PROCEDURES

PURPOSE

This procedure establishes direction for completing the Pre-Entrance History and Physical Examination form and the Hillsborough Community College Health Record Update form for students in the Health Science/Nursing programs at Hillsborough Community College.

PROCEDURE

1. Students enrolled in the following Health Science programs will complete the Pre- Entrance History and Physical Examination form for the Health Science programs and the Hillsborough Community College Health Record Update form:
   
   1. Paramedic;
   2. Radiation Therapy Technology;
   3. Medical Clinical Dosimetry Management;
   4. Radiography;
   5. Nuclear Medicine Technology;
   6. Diagnostic Medical Sonography Technology;
   7. Human Services;
   8. Radiation Therapy Specialist;
   9. Medical Clinical Dosimetry;
   10. Diagnostic Medical Sonography Specialist;
   11. Occupational Therapy Assistant.
2. Students enrolled in the Ophthalmic Dispensing Program will complete only the history section of the Pre-Entrance History and Physical Examination form for the Allied Health programs.
3. Students enrolled in the Associate Degree Nursing program will complete the Pre- Entrance History and Physical Examination form for the Nursing program and the Hillsborough Community College Health Record Update form.
4. The Pre-Entrance History and Physical Examination form must be completed, mailed and received at the Office of the Dean of Health, Wellness and Sports Technologies prior to or on the first day of classes. If the form is not received on the first day of classes, the student is required to report to the Dean of Health, Wellness and Sports Technologies for authorization to continue in the program.
5. Students who do not submit a completed form by the deadline noted above without approval by the Dean of Health, Wellness and Sports Technologies will be dropped from the program.
6. The Hillsborough Community College Health Record Update form must be completed, mailed and received at the Dean of Health, Wellness and Sports Technologies prior to or on the first day of classes of the second year of the program or the student will not be permitted to enter the clinical setting.
7. Students who are unable to demonstrate good physical health status may reapply when the problem is corrected or obtain a second opinion from a physician selected/designated by the College.
8. The information contained in both the Pre-Entrance History and Physical Examination form and the Hillsborough Community College Health Record Update form is confidential and may only be released following receipt of written authorization by the student.

   The Dean of Health, Wellness and Sports Technologies may have access to the student’s health record without prior written consent by the student.

9. The Dean of Health, Wellness and Sports Technologies will ensure that a cover letter is included with the Pre-Entrance History and Physical Examination forms and the Hillsborough Community College Health Record Update form, outlining the dismissal guidelines noted above.
